There are three routes to apply for Short Breaks.

1. Referral from the Disabled Children's Team

2. Referral from Aiming High

3. Parent / Carer Referral and Referral from outside Borough.

If you are interested in applying for a place of Short Breaks then please email  the Short Breaks Referral Form and email to Kim West, Short Breaks Coordinator on email admin@shortbreaks.org.uk

There are several funding stream available to help your child or young person access the Short Breaks Respite.
